1. Make sure that your old passport:

- Was issued within the last 15 years.
- You were at least 18 years of age at the time.

NOTE: 

Your previous passport will be returned to you with your new passport.

2. Complete application form DS-82:

Complete all required fields and print out the application. Be sure to sign and date your application.

3. Fee

Standard: $110+$300 / per passport (18-20 weeks)

Expedited: $140+$400 / document(8-12 weeks)
